我应该何时去做癌症筛查？

癌症可影响所有年龄段的男性和女性，虽然有些人可能比其他人面临更高的风险，但我们都应通过接受建议的筛查测试来保持警惕。
被诊断出患有癌症是一件令人恐惧的事情，这也是为什么有些人逃避筛查的原因。然而，忽视警示信号和拖延急需的检查会让癌症生长和扩散，增加治疗难度。
根据世界卫生组织（WHO）的数据，2020 年大约有 1000 万人死于癌症。然而，估计有 30 - 50% 的癌症是可以通过避免风险因素和采取预防措施来预防的。
做到这些的一个好方法就是改变生活方式，降低患癌症的风险。这包括避免吸烟和酗酒、均衡饮食、多做运动。接种乙型肝炎和人乳头状瘤病毒（HPV）疫苗等保护措施也有助于预防肝癌和宫颈癌。
为了支持您选择健康的生活方式，筛查对于早期发现癌症非常重要，可以在症状出现之前甚至在癌变前分期发现癌症。如果不进行筛查，有些癌症要到晚期才能确诊，从而限制了治疗方案，影响了存活率。
在本文中，我们将介绍新加坡最常见的癌症、不容忽视的癌症症状和体征，以及建议进行哪些筛查。
新加坡最常见的癌症有哪些？
结直肠癌
这是男性中最常见的癌症，占 16.3%，女性中占 12.9%，位居第二。开始时是非癌性（良性）生长，称为息肉，有时会发展成癌症。这一过程可能需要数年时间，因此有时间通过一种或多种筛查检测进行早期发现。
晚期大肠癌的体征和症状包括：不明原因的体重减轻、排便习惯改变、腹部疼痛、抽筋、粪便中或粪便上有血液、腹泻、便秘或排便后仍感觉肠道没有完全排空。请记住，大肠癌的早期癌症预警信号是不会出现的，这些症状通常出现在大肠癌晚期患者身上。
要检查大肠癌，粪便潜血试验（FOBT）可以查看粪便中是否有血液迹象。最常用的粪便隐血试验是粪便免疫化学试验（FIT）。如果检测结果呈阳性，建议进行结肠镜检查，以检查大肠内壁是否有息肉并将其切除。检测结果呈阴性的人需要每年重复检测。
建议 50 岁以上无症状者进行筛查。对于有大肠癌家族史的人，筛查应从更早的年龄开始。
乳房癌症变
乳腺癌是新加坡女性罹患癌症的主要原因，发病率为 29.7%。每年约有 2000 名女士被确诊。
乳腺癌的症状和体征包括乳房任何部位疼痛、乳房大小和形状改变、乳房或腋下肿块、乳房某个部位增厚/肿胀、乳头部位或乳房皮肤发红/脱皮、乳头部位疼痛或乳头牵拉、乳头溢液，包括血液（非母乳）。
虽然 80 - 85% 的乳房肿块最后证明是非癌性的，但任何肿块或异常情况都应进行检查，以排除癌症的可能性，因为早期治疗提供了成功治疗的最佳机会。
除了自我检查乳房组织或皮肤是否有肿块和变化外，最有效的乳腺癌筛查方法是进行乳房 X 线照片检查，它可以发现在自我检查时摸不到的小肿块。然后，医生可能会建议您进行活检，检查肿块是否癌变。
建议 40-49 岁的女士每年做一次乳房 X 线照片，50 岁及以上的女士每两年做一次。
前列腺癌
这是新加坡男性中第二常见的癌症，发病率为 16.8%，随着年龄的增长，患病风险呈指数增长。在早期阶段，没有任何症状，而排尿困难或血尿等症状通常出现在癌症晚期。
前列腺癌的其他症状和体征还包括尿流减少、精液带血、骨痛、体重减轻和勃起功能障碍。
虽然前列腺癌通常发展缓慢，但仍有 25% 的病例在晚期才被发现。通过筛查，早期前列腺癌患者的 15 年存活率为 90%；相比之下，晚期前列腺癌患者的 5 年存活率仅为 40%。
建议 50 岁及以上的男性进行前列腺癌筛查，如果有家族史，则应更早进行筛查。前列腺特异性抗原（PSA） 测试如果水平较高，通常提示前列腺增大或前列腺癌。如果怀疑患有前列腺癌，医生会建议您进行前列腺活检，切除组织样本，检查是否有癌变细胞。
肺癌
在新加坡，肺癌主要有两种类型，分别占男性病例的 13.5% 和女性病例的 7.9%。它们分别是非小细胞肺癌（NSCLC）和小细胞肺癌（SCLC）。
肺癌的症状和体征包括不明原因的体重减轻、嗜睡（总是感到疲倦）、持续咳嗽、气促、喘息、咯血（咳血）和胸部疼痛。
2014 年至 2018 年期间，新加坡共确诊近 8000 例肺癌，其中 85% 的病例为 NSCLC，包括腺癌、鳞状细胞癌和大细胞癌。其余15%的病例为SCLC，这种癌症更容易快速生长和扩散。
肺癌筛查采用低剂量电脑断层扫描，建议年龄在 55 - 74 岁之间、吸烟 30 包年或以上*的人群进行筛查。

我应该什么时候去做癌症筛查？
说到患癌症的可能性，知情并积极主动地接受治疗有助于早期发现癌症，此时治疗可能最有效。这可以提高存活率，改善生活质量，因此，不要等到男性和女性癌症的常见症状和体征出现时才进行癌症筛查。
